The Racer Forms page will archive all necessary forms that racers will need to race in the MXR Racing Series. All forms are in Portable Document File (PDF) format which are readable with Adobe Acrobat Reader which is a standard install on most home computers. Click on the appropriate form and print out on a personal printer.

MXR Racing Membership
• MXRmembership.PDF -
MXR Minor Waiver Form - If you are under 18 years old, you will need to submit an original, notarized, minor waiver form.
• MXR_minor_waiver.PDF Faxes will not be accepted. We must have a signed and notarized original form, plus a copy of minor's birth certificate on file the Wednesday before the scheduled event.
MXR Power of Attorney Form - This form is required when a minor (under 18) cannot be accompanied by a parent/guardian to the event. It gives the power for another adult to sign for them. This form must be notarized, completed and brought to the track at driver check in.
